Prior to scheduling your first appointment, we ask that all new patients please fill out the new patient questionnaire and consent forms. To get started, please TEXT our office staff, Madison and Alesha, at (865) 539-2221 with your name and an email address so we can send you new patient forms. These forms allow us to learn a bit about you prior to your visit and use our time together in the most helpful manner. You will complete the new patient forms online in our secure electronic medical record. Please text us again once you have completed the forms and submitted them in full and we will reach out to you to for scheduling.
Please note, we are private pay and do not accept insurance. We want to ensure our patients are aware of this prior to scheduling and will confirm pricing with you when you reach out to us.
On the day of your first appointment, please bring a complete and updated medication list, a form of personal identification, your health insurance cards and any pertinent medical records or other information that would be helpful for us to review. It can be helpful to bring a close friend or family member with you to your first appointment, especially if you have any difficulties with memory.
A new patient appointment lasts approximately one hour. We will review your past medical and psychiatric history, review your medication and discuss your presenting concerns at length. We will then work together to create a treatment plan which can include prescribing medication, psychotherapy, or other therapeutic interventions. There are times when we may need to order other tests (lab work, urine tests, brain imagining, sleep studies, etc) to better understand your condition. The frequency of your follow-up appointments will be decided at each visit; we will work together to determine how often we should meet.
You will be expected to pay for your appointment at the time of service. We do not bill patients. We accept payment with cash, check,Visa or Mastercard.
Fees are as follows:
New patient – $450.00
Return patient- $175.00
We are not in-network with Medicare or any other commercial insurance companies. For patients with private insurance, we will provide you with the necessary documents to file with your insurance company for our of network reimbursement at your request. After you reach your out of network deductible, you may receive reimbursement for part of your fees. By law, we are not allowed to file Medicare or TennCare for any type of reimbursement.
